Dear Patients,

IHOPE Clinic regretfully informs you that Angie Albert MSN/APRN/PMHNP will no longer be
working for IHOPE Clinic as of 10/2/22. Ms. Albert will not be available to attend to you after
that date. No appointments will be available after 9/5/22. Active patients who have showed
up for their last appointment will receive 30 days of medications from 9/2/22 in order to have
time to get into another provider. If an emergency is present, patients as always should call
911 or go to the nearest emergency room.

Since your condition may require additional care, we suggest that you arrange to place
yourself under the care of another doctor. Referral resources are also provided below. To
assist you in receiving the care that you need, IHOPE will be happy to provide your new
doctor with copies of the necessary records from your file. On your patient portal you will find
a coordination of care document with your medications and diagnosis for your next provider.
Also you can get a current full list of all your medications from all your providers simply by
contacting the pharmacy you use and requesting one. You can access the portal by going to:

If after trying to login your portal link is no longer active you may email Ihope your current
email address to portal@ihopeclinic.com.

We regret that Ms Albert and IHOPE Clinic will not be able to continue to serve you. Our
years here have been filled with many rewarding experiences and memorable patients.
